Athletic Clearance Forms

Click here to access athletic clearance forms.    If your student is planning to participate in athletics at Newport Harbor, it is mandatory for them to have the following documents completed and submitted to the athletic office prior to participating in a sport or summer program.  Please be sure to attach a copy of your student's current medical insurance card. If you do not have medical insurance, you can obtain coverage for your student for school athletic events from Myers-Stevens. Click here for information/application.  Make sure you initial the IV Insurance Certification section.

The NHHS Athletic Department in association with the NMUSD, CIF, the Josephson Institute of Ethics and the community share the goal of teaching the importance of character. All of the athletes at NHHS strive to understand the importance of being a “Person of Character” by living according to the Six Pillars of Character. All of the coaches at NHHS have been trained with the Josephson objectives as noted below:
